Moon rules Cancer, so this card is not describing an outside force. It is describing Cancer to itself, which is why there is no distance in it. Upright, this is happening out in the visible world, where other people can see it and respond to it.
When reading about health through a Cancer lens, the High Priestess card says that you feel like your body holds what you will not say out loud, rest feels like abandonment (or like you are letting people down), and you know something is wrong before any test confirms it. This has either made you chronically tense and dismissive of your own pain, or taught you to listen when your gut tells you to stop. The body has been sending the same signal for months and nobody has bothered to listen. The Cancer blind spot in it is being seen as a burden, or as someone whose needs are too much.
In practice that can look like the body making the decision for you, or a genuine reset. Whatever form it takes, it is happening in the visible world where other people can see it.
