Some early depictions of this card showed him as a juggler balancing the many things in life. However, the magician here is perhaps invoking Merlin more than a juggler. The infinity symbol above his head has been used in mathematics since the 17th century, but the symbol is often referencing the ouroboros (a snake eating it's own tail) or the inner magical knowledge

Mercury rules Gemini, so this card is not describing an outside force. It is describing Gemini to itself, which is why there is no distance in it. Upright, this is happening out in the visible world, where other people can see it and respond to it.
When reading about health through a Gemini lens, the Magician card says that you feel like your body is a problem to research, something you understand in theory better than practice, and a thing you will address once you find the perfect plan (or once you finally stop waiting). This has either made you knowledgeable and eventually proactive or left you reading about the symptom for months instead of calling the doctor. The body has been sending the same signal for months and nobody has bothered to listen. The Gemini blind spot in it is stillness, silence, the moment when talking stops and you have to feel what you actually feel, the weight of having chosen one thing and not the other.
In practice that can look like the body making the decision for you, or a genuine reset. Whatever form it takes, it is happening in the visible world where other people can see it.
