Saturn sits outside Leo's usual Sun register, so this one arrives as unfamiliar weather. Upright, this is surfacing whether or not it was invited.
When reading about health through a Leo lens, the World card says that you feel like your body is something you push and recover from in private, something to be proud of (or to hide the cost of), and something that should not betray you publicly. This has either built real strength and self-knowledge, or left you managing an injury or exhaustion you cannot admit to anyone.
