Saturn sits outside Scorpio's usual Pluto register, so this one arrives as unfamiliar weather. Upright, this is surfacing whether or not it was invited.
When reading about health through a Scorpio lens, the World card says that you feel like your body is something you ignore until the problem is undeniable, a machine you are behind on maintaining (or refuse to examine closely), and a frontier you do not trust yourself to explore. This has either led to you discovering something serious late, or to you becoming obsessively knowledgeable about the one thing you do decide to fix.
