Pluto works on a longer clock than Neptune-ruled Pisces is used to. Whatever this is, it will not resolve on Pisces's preferred timeline. Upright, this has turned inward, to the place where the outer tools stop working.
When reading about health through a Pisces lens, the Death says that you feel like your body is a signal you can ignore, pain is something to numb or postpone until it screams, and admitting you're suffering feels like complaining or demanding too much. This has either kept you functional through crisis by numbing out completely, or meant that by the time you pay attention the problem has grown teeth. The body has been sending the same signal for months and nobody has bothered to listen. The Pisces blind spot in it is the moment they have to stop being useful and admit they're drowning.
In practice that can look like the body making the decision for you, or a genuine reset. Whatever form it takes, it is turning inward, where the usual moves stop working.
