Mars rules Aries, so this card is not describing an outside force. It is describing Aries to itself, which is why there is no distance in it. Upright, this is happening out in the visible world, where other people can see it and respond to it.
When reading about health through a Aries lens, the Chariot card says that you feel like your body is a project to finish, a warning sign you can think past (or ignore), and something that only matters if it is slowing you down. This has either made you the person who outtrains their doubt and builds real resilience, or left you injured and surprised that your will could not override what was actually broken.
